Role

Who You Are

You are a dedicated Occupational Therapist with a Degree or Diploma in Occupational Therapy and HCPC registration. You have experience in adult social work or adult health environments, and are well-versed with safeguarding policies and procedures. You understand the Care Act 2014 and the national health and social care agenda. You act in accordance with the Employee Code of Conduct and Council Values, and you are proactive in promoting equality and preventing discrimination. You are flexible and mobile, able to work from various locations as needed.

What the Job Involves

The role involves providing Occupational Therapy interventions for adults to maintain their independence at home. You will conduct holistic, strength-based assessments, developing personalized goals and reablement plans. Your responsibilities include supporting hospital-to-home transitions, preventing hospital admissions through community reablement, assessing and arranging equipment adaptations, and undertaking complex risk assessments. You will actively involve service users and carers in the planning and use of Direct Payments. You will demonstrate assistive technologies, ensure accurate case records, supervise OT Assistants, and collaborate with other professionals for seamless service delivery. Additionally, you will engage in ongoing personal development and adapt to evolving Adult Social Care policies.
