You have excellent communication skills and the ability to build relationships with multidisciplinary teams, social care services, care providers, and equipment suppliers. Your approach is flexible and responsive to meet urgent needs and support the duty function.
What the Job Involves
Your role involves providing therapeutic interventions within patients' homes and community settings to support independence and prevent unnecessary hospital admissions. You will conduct comprehensive assessments, develop intervention plans, recommend assistive equipment, process Disabled Facilities Grants, and participate in housing assessments. Your responsibilities include supporting admission avoidance, acting as a Trusted Assessor, managing a varied caseload efficiently, and participating in a weekly duty rota for urgent visits and queries.
