What the Job Involves
Your role involves managing a caseload, conducting needs assessments, and developing care and support plans for adults in line with the Care Act 2014. You will work autonomously, supporting people to build networks, and address safeguarding concerns. Additionally, you will provide support to other care practitioners and work collaboratively within a multi-agency setting, which may require travel outside the county.
